- What is the Registration deadline?
- We would like for all registrations to be completed and in NO LATER than Wednesday, September 4th, 2024
- Your prompt attention in this matter is greatly appreciated!
- What are the fees associated with the 2024-2025 Forming Young Disciples Formation Program?
- $75 per child
- $200 family max
- Payment in full expected at the time of registration either online or pay by check
- St. Anne Parish does not prevent participation based on your ability to pay tuition. To make payment plan arrangements or inquire about possible tuition assistance please let us know.
- I am serving as a catechist (or am willing to serve in this capacity), is there a tuition discount?
- YES there is! We truly appreciate your willingness to serve in this important ministry!
- We will gladly waive the tuition fees for any associated children.
- Please be sure to contact John Schmitt or Lee Ann Lella in the parish office if you are willing to serve in this role!

- What about Sacramental Preparation for 1st Communion and Confirmation? How do we register our son/daughter for those processes?
- A seperate notice will go out in late August/Early September announcing the details of these programs
- Preparation for both Sacraments will begin in October

Online Registration
1. IF needed, create an account at My Own Church or login to your account you created previously (or change your forgotten password) in order to access your parish database family record.
For registered parishioners of St. Anne Parish, we are using the My Own Church portal to register your child(ren) for the Young Disciple Formation Program. Please complete the following steps to register your son/daughter. If you are NOT a registered member of the parish, please call or stop in to the parish office for further information.
PRO TIP: Unfortunately, the registration system is NOT mobile friendly. Be sure to complete the process on a laptop or desktop computer.
STEP ONE-Log in OR create your account
2. PLEASE NOTE this step DOES NOT register your child/ren for our Disciple Formation Program, it only logs you in/creates an account for you/your family
STEP TWO-REGISTER YOUR CHILD/REN FOR DISCIPLE FORMATION
1. Once logged in to the account created above, click on "Disciple Formation" on the black side bar menu.
2. Review, and if needed, update your current family details, especially your household's primary contact information.
5. Select the name of each child you would like to enroll
5. Select the appropriate grade per child
6. Please specify any special learning needs or Health or Medical Needs
7. Select the appropriate session preference for each child. You can choose from the Wednesday Evening Session OR the Homestudy/self-guided session. PLEASE NOTE the Homestudy option is only offered for grades K-5.
8. Click "Save Student"
9. Repeat steps 4-7 for any additional students
10. When finished, click "Submit Registration"
11. This will then show you a confirmation screen, please verify all information is correct. Please select the blue "Pay Now" button to submit your tuition payment. This will then take you to the OSV parish online giving portal to process your online payment.
12. You will receive a confirmation email stating your registration was received and is pending final approval by the parish office. Once we approve and/or assign your child(ren) to a specific group, you will then receive a final confirmtion email.

Registration Option B
Having issues with the online registration process? Prefer to register via paper form? You have three alternative options:
- Download and print a form to write in your registration and drop off/mail the completed form to the parish office c/o John Schmitt
- Stop by the parish office to complete a paper form
- Pick up a registration form in the Main/West Entrance of Church
